Refund Request Form

This form MUST be completed fully and submitted to us by clicking the "Submit" button at the bottom of the page. As it states in our FAQ's we follow a strict set of return policies to protect the customer and avoid any confusion later. Once you submit this refund request, our Customer Service department will review and contact you via email within 2-3 days. Not all refund requests are eligible for a refund. If a refund is approved you will be responsible for the shipping costs to return the item to our warehouse and the original shipping costs will be deducted from your total refund amount.
